This facility is well worn. Exterior carpeting is old and dirty. Entry door not well sealed. Bath sink was not cleaned properly in the bowl. Shower soap and shampoo pumps (in shower) were empty. Bottom of shower door has mold. A white bed pillow had black stains. The other bedding was fine. Refrigerator ...
